When scheduling an appointment, be sure to provide insurance card information, along with a prescription from your doctor, as required. Bring your card with you to the appointed session. Most insurance policies are accepted, and co-payment or co-insurance is due at the time of service.

Labor and Industries

The first 6 treatments from L&I do not require approval. For your first appointment, provide your case number, and bring a copy of the L&I Report with diagnosis codes, and a prescription from your doctor.

Personal Injury

For your first appointment, provide your case number, a prescription from a doctor, proof of auto insurance with PIP and your adjusters name & phone number, as well as your attorney’s contact information, if applicable.
